Zytlaly Zamora has been chosen as one of Partners’ two Employees of the Month for July. She is a Chronic Care Management Coordinator (CCM) who has been working at Partners since August 2020. In this role she conducts case management in partnership with a team of outside physicians and coordinates their care with the Multipurpose Senior Services Program (MSSP) team. She has also stepped in to assist the MSSP program with Spanish translation, reassessments, vaccinations, and calling participants directly. She has become a resource for training participants to use technology, assisting with CCM telehealth devices and the LA Google Home project. Zytlaly is a Certified Medical Assistant with prior clinical experience in primary care, dermatology, and dialysis clinics. She continues to expand her capabilities, both through her work at Partners, and at Los Angeles Mission College where she is studying for a Bachelor of Science in Nursing degree. While her knowledge and skill have been invaluable, she has earned recognition within the organization for her flexibility and enthusiasm in taking on new challenges. Zytlaly is a highly valued team member who lifts the performance of all and is a very valued resource within Partners. Congrats on the honor!
