Service Story – Social Care Services Helping a Senior Live Independently at Home

82-year-old Nick lives with diabetes, a history of prostate cancer, and has a history of falls. Living alone with no support system is challenging and he experiences limitations in his ability to perform basic daily activities.

Nick enrolled in Partners MSSP, and they immediately helped connect him with tools and resources like In Home Supportive Services (IHSS), CalFresh (food stamps), and a Rollator Walker.

The MSSP Care Manager helped Nick apply for IHSS hours and he now receives help several times a month with household chores, cooking and cleaning, tasks that once were a burden to him.

To better manage his diabetes, he receives support with his meals and groceries through the CalFresh program. CalFresh provides food stamps to assist low-income individuals, like Nick, in purchasing nutritional food items to meet his needs.

MSSP was also able to connect him with a medical supply store, via Mercy Plaza Respiratory, that provided Nick with a rollator walker to help with his balance and stability and reduce overall falls.

Thanks to the assistance of MSSP services, Nick can continue to live safely alone and comfortably in his own home.
