Congratulations to Patricia “Paty” Gomez for receiving Employee of the Month for October 2022. Paty was one of the first Care Coordinators hired at Partners in Care Foundation, and essential to the launch the Health Homes Program in 2019. Since then, she has become a core team member, always welcoming new staff, and offering her support.
Every week she reaches out to the team to offer a positive thought or affirmation to start the week on a good note. She has taken a personal investment in the wellbeing of her team, dubbing herself the “HHP Mama Bear” and making herself available for tips, pointers or just a supportive ear. When the pandemic hit and the HHP weekly meetings couldn’t be held in the office anymore, Paty helped to create “Let’s Do Lunch” a virtual weekly lunchtime Zoom meeting for the team to enjoy lunch together.
As HHP evolved into Enhanced Care Management (ECM) in January, Paty joined the ECM Clinical Team and helped to develop a position unique to Partners, the Substance Use Disorder Navigator. As a Substance Use Disorder (SUD) Navigator Paty uses her special “Paty Magic” to engage some of our most challenged members and help them find their way to treatment and recovery. This role has allowed her to offer her unique skills to the team in an innovative way that has helped to make Partners a lead ECM provider in Southern California.
