The National Committee for Quality Assurance (NCQA) has awarded Partners in Care Foundation a three-year Case Management Accreditation for its complex case management work with the Multi-Purpose Senior Services Program (MSSP). MSSP provides community-based care management and evidence-based self-management programs, aimed at delaying or even preventing unnecessary nursing home placement.
This is the second NCQA Accreditation that Partners has received. At the time the first Accreditation was awarded in 2015, Partners was only the second community-based organization in the country to achieve the prestigious NCQA seal. Widely regarded as representing the gold standard for the assessment of and reporting on healthcare provision, NCQA is a private, non-profit organization dedicated to improving health care quality.
“Following our initial two-year Accreditation we have significantly increased our quality improvement activities throughout our entire organization,” explained Tahirah Farris, Director, Quality and Metrics
at Partners in Care. “This second survey process focused not only on confirming that Partners continues to operate according to NCQA’s strict guidelines for the highest quality of case management, but also on reviewing evidence that we have improved the quality of our operations since 2015. The evidence we presented to NCQA demonstrated our ability to adapt to the changing healthcare environment and our population’s needs, while still ensuring we provide the best program for our clients.”
