San Fernando, CA – Today it was announced that Dan Swayze, DrPH, MBA, MEMS of Pittsburgh, PA, has been hired by the Partners in Care Foundation as its new Chief Operating Officer (COO). The agency, based in San Fernando, California, is a nationally known social care services provider and center for social care practice innovation.
According to June Simmons, the agency’s President, and CEO, “We are excited about Dan joining our organization. He brings a wealth of experience gained from over thirty years of health care/social care alignment management with vulnerable populations and patients in both home and community-based settings.”
Swayze served as Vice President of Community Services at UPMC Health Plan in Pittsburgh, PA, where he has led a team of more than one hundred multidisciplinary, in-home care managers. Other work highlights include:
Swayze has been the recipient of numerous awards, including the Lifetime Achievement Award from the Academy of International Mobile Healthcare Integration (2024) and the Excellence Award from the Pittsburgh Emergency Medicine Foundation (2023).
In addition to his professional work, Swayze has volunteered extensively with several Pennsylvania non-profit agencies, including board services on the Greater Pittsburgh Community Food Bank and UPMC Passavant and UPMC St. Margaret Hospitals among several others.
He has earned a doctorate in Public Health and two Masters Degrees (MBA and Masters of Emergency Medical Services). Swayze is married to a wife of thirty years, and together they have raised one son and one daughter.
