Partners in Care Foundation has begun operating its quality accredited Multipurpose Senior Service Program (MSSP) in Santa Barbara County, replacing the previous provider, CenCal Health. MSSP helps frail seniors to remain at home, delaying or avoiding entirely, inappropriate and costly nursing home placement through the provision of services and supports necessary for independent living.
The high quality of Partners work in complex care case management, specifically MSSP, was recently recognized with a two-year Accreditation from the National Committee for Quality Assurance (NCQA), making Partners only the second community-based organization in the nation to receive the NCQA seal.
Partners in Care is the largest MSSP provider in California, with 15 years of experience operating the program in LA County and Kern County. The organization’s team of highly-trained MSSP Care Managers visit the senior at home to conduct a health and psychosocial assessment. They then work with them and any family caregivers to develop comprehensive care plans, obtain critical resources and services, and ensure participation in essential programs that will foster self-sufficiency and assist the person in his or her daily living.
