Congratulations to Sheila Ochoa for receiving Employee of the Month for the month of April. Sheila joined Partners in Care Foundation last year and is our Engagement Center Supervisor. She oversees outreach and engagement for various Partners programs. She works on surveys for both LTSS and Network programs including HCBA, ALW, ECM, Blue Shield, Providence, Arrowhead, and MSSP. In addition, she completes outreach, enrollment, and assessments for Network’s Anthem care coordination program and the ECM program.
During her time here, she has discovered ways to improve the quality of interactions with clients by doing research and setting action steps towards improvement. She has also worked diligently to identify and resolve many issues with the dialer and case management software, both the internal and external payor driven systems.
Over the past 2 months, she has onboarded the Whole Person Care transition to CalAIM’s ECM program outreach and has launched an ad hoc informational campaign for L.A. Care members to increase their awareness of COVID vaccine resources, all while maintaining her regular outreach requirements. This project was a tremendous amount of work – from updating and testing the dialer, onboarding 5 additional staff in a 2-week period, creating all training materials – while continuing to make sure her current team is fully supported.
We have feedback from both IT and ECM Leadership that although new in her role she excelled as a leader. She communicated well with ECM leadership to report members with mental health or housing crisis to ensure they received immediate care. In addition, she played a vital role in reporting outreach data for Health Net’s Whole Person Care project which helped provide better understanding of the types of outreach that are most effective in driving return calls to the Engagement Center. She is a great addition to the Partners team!
