A Journey of Healing: Restoring Yasmin’s Independence and Dignity

At 70, Yasmin once lived a vibrant life. But with weakened legs, joint pain, and the lingering effects of a recent fall, her daily routine had become a struggle. As her body weakened, so did her confidence, intensifying fears about losing her independence and the security of her own home. Recognizing her need for additional social support, Yasmin’s hospital social worker referred her to Partners in Care Foundation.

With the help of a dedicated care manager, Partners conducted a full health assessment and saw that Yasmin needed more than medical care— she needed tools and resources to safely move around her home and regain her sense of independence. Through the state funded Multipurpose Senior Services Program (MSSP) — a lifeline for individuals aged 65 and older who are at risk of nursing home placement— Yasmin was provided a seated walker to help her move safely around her home and accessible transportation to get her to physical therapy appointments. These interventions significantly improved Yasmin’s life, helping her regain mobility, confidence, and strength, which reduced her risk of falls and allowed her to enjoy daily activities and engage with her community again.

Through Partners’ Community Care Hub (CCH), which collaborates with an extensive network of community-based organizations (CBOs) across California, Yasmin’s needs were met with care and dignity. The CCH is a powerful community-focused network that partners with hospitals, health plans, and public agencies to support those facing health-related social needs across the state.

When her doctor recommended additional comforts, such as an elevated pillow and memory foam topper, Yasmin worried about her limited budget. But thanks to the Partners’ Urgent Needs Fund, supported by generous donors, these essential items were provided—allowing Yasmin to rest comfortably, relieve pain, and focus on her healing.
