Congratulations to Krystal Ford for receiving employee of the month for January 2023. She started at Partners in July 2020 as a Care Coordinator in the Health Homes Program (HHP) and she has been key in our transition into Enhanced Care Management (ECM). Her background includes a wealth of medical knowledge that she readily shares with her participants and coworkers, making her an invaluable asset to the team. Krystal’s participants always come to appreciate and depend on her consistent support. She is equally valuable to her coworkers—she is a go-to for community resources and most new hires shadow her to see how care coordination is done best.
