Congratulations to Nayla Zoueihid for being honored as Employee of the Month for April 2023!
She started as a Care Coordinator with ECM at the end of 2020. Nayla quickly distinguished herself as a strong communicator and is extremely organized. It was also apparent that she had a natural affinity for helping people who struggle to stay healthy. These earned her the specialized role of Hospital Navigator which is critical to the ECM team.
As Hospital Navigator, Nayla is responsible for monitoring the hospital admissions and discharges of the almost 900 ECM members across southern California. When a member is admitted, she notifies Partners’ ECM staff and provides a Transition of Care plan. Nayla has stepped into this important role with dedication and humility while continuing to support her own caseload of members.
Her pleasant demeanor, quick mind and “can-do,” attitude make her a pleasure to work with and make us all thankful to have her on our team.
