Congratulations to Delia Lezama for receiving Employee of the Month for September 2024.
Delia Lezama is a Coach for our Short term services and supports (STSS) program and has been a part of the Partners in Care Foundation family for 11 years.
Delia is knowledgeable, compassionate, a personable person who is dedicated to helping those in need. She tackles anything thrown her way with grace.
Delia has worked with many programs providing exceptional care to our members. Currently Delia is part of our Anthem/Elevance team, a team that provides care coordination services to their members across the state. Delia is quick to find resources for members regardless of where they are. Delia is an asset to our program with a wide understanding of Anthem Insurance, the platform used to conduct her work and most of all, to the needs of the members.
Delia is also an employee that her team can depend on. She is quick to share her knowledge with other care coordinators, helping them be more successful in their role. She is always available to assist with clarification, and welcomes her team members to shadow her workflow during their onboarding process.
