Congratulations to Leticia Martinez for being honored as Employee of the Month for March! Leticia is a Care Coordinator that joined Partners in Care Foundation a year ago. She played a key role in the transition of the Health Homes Program to our Enhanced Care Management by training newly hired Care Coordinators during a significant staffing expansion. She also led other projects such as the development of a potential mentor program for Care Coordinators.
Leticia met challenges throughout her time at Partners and has been able to find creative solutions on her own. She carried a large caseload for 1.5 months during a staff shortage and assured her members never had a gap in their care. Her positivity, openness to trust the process and ability to create strong boundaries makes her a strong Care Coordinator and a pleasure to work with. We are excited to continue working with and seeing Leticia grow professionally. Congratulations on this recognition!
