We are delighted to announce the promotion of Martha Delgado as Director of Care Coordination as part of the STSS and Hub team. Martha has been working in the social services field since 2004. Since the beginning of her career, she’s been passionate about helping the older adult population.
She’s joined Partners In Care Foundation in 2015 and started as a Coach for the Short Term Services and Supports team. She was then promoted to Supervisor where she worked closely with our Hub network members.
Martha is a mother of two. Her eldest is 15 and youngest is 9. she spends most of her weekends watching them play sports (baseball / softball) and the evenings are spent at the park. Martha’s favorite pass time is watching them play / grow and she occasionally enjoys crocheting.
We look forward to seeing Martha continue to thrive in her new role.
