The City of West Hollywood, in collaboration with Partners in Care Foundation, West Hollywood Community Housing Corporation and Jewish Family Service – Los Angeles, has been awarded the Social Change and Diversity Award by the American Planning Association (AMA) California Chapter, Los Angeles Section.
The award is for the City’s Aging in Place/Aging in Community five-year strategic plan, which aims to improve the quality of life and well-being of older adults living in West Hollywood. The plan is designed to help seniors maintain health and well-being, enabling them to remain in their homes as they age by providing supportive services that foster independence. Partners in Care Foundation, a national leader in developing and spreading high value models of community-based care and self-management, provides technical assistance and training for individuals interested in leading health self-management workshops for older adults.
Commenting on the award, Andres Gonzalez, Program Director for Health Self-Management Services at Partners said, “We’re very happy that the City of West Hollywood has been recognized for its work to enhance the lives of its seniors. We’re excited to be working with the City to provide Arthritis Exercise classes, Healthier Living classes – which help people with chronic conditions to manage their health – and Matter of Balance classes, which help people learn how to avoid falls. These evidence-based workshops have been demonstrated to help support optimal health for people with a wide range of conditions, promoting independent living for longer.”
