Partners’ MSSP Referral Form

Partners in Care Foundation is accepting enrollment for its Multipurpose Senior Services Program (MSSP). MSSP acts as an alternative to nursing facility placement by providing a care management service that helps older adults, who are eligible for Medi-Cal and at risk of institutionalization, remain living safely in their own homes and communities through coordinated community services and support systems. 

To be considered eligible for the Multipurpose Senior Services Program, participants must:

• Be 65 years of age of older
• Live within a sites service area
• Be in need of care management services
• Be a Medi-Cal recipient
• Be at risk of placement in a skilled nursing facility
