Partners conducts evidence-based workshops for low-income, community-dwelling older adults with challenging chronic health conditions such as diabetes, heart disease, arthritis, or chronic pain. Most community wellness workshops are available online, in-person, or by telephone. In English or Spanish.
Through federal grant funding, Partners has built the California Healthier Living Coalition. The California Healthier Living Coalition is recognized by the National Council on Aging as a statewide falls prevention coalition in California, as it allows for sharing of best practices among coalition members, streamlining of tools and workflows, and data sharing.

Chronic Disease Prevention and Management Program
CDSME programs provide older adults and adults with disabilities with education and tools to help them better manage chronic conditions such as diabetes, heart disease, arthritis, chronic pain, and depression.
Workshops Include:

- Tomando Control de su Salud.
The Falls Prevention Programs include evidence-based falls prevention workshops that have been proven to reduce falls, fear of falling, and fall related injuries in older adults.

- Bingocize
- Tai Chi for Arthritis
- Enhance Fitness.
Physical activity programs focus on effective ways to help older adults with common conditions like arthritis get more active.
