Partners’ Comprehensive Medication Review Program
Medication-related problems (MRPs) are a leading cause of emergency room visits, rehospitalizations, and other serious health complications. Studies show that medication errors account for over 3 million hospital admissions annually, resulting in $21 billion in healthcare costs each year. The good news is that HomeMeds℠ offers a powerful solution for identifying and preventing these issues through a comprehensive medication review.
HomeMeds℠ is designed to be easily integrated into in-home visits conducted by care coordinators, community health workers, or other non-clinical staff, providing a simple yet effective way to monitor and optimize patient medications outside the traditional healthcare setting.
With its proprietary algorithm, HomeMeds℠ can quickly identify:
- Hazardous Drug Interactions: Prevent harmful adverse effects by detecting dangerous interactions between medications.
- Therapeutic Duplications: Eliminate unnecessary duplications in treatment, leading to better patient outcomes.
- Psychotropic Medication Risks: Identify fall risks and confusion caused by inappropriate psychotropic medications.
- Cardiovascular Medication Concerns: Recognize potential cardiovascular issues tied to certain medications, improving patient safety.
By addressing medication-related issues early, HomeMeds℠ has been shown to significantly reduce hospital readmission rates and improve overall patient health, making it an essential tool in promoting medication safety.
How HomeMeds works:
- License HomeMeds & Conduct an In-Home Assessment
Organizations license HomeMeds for use by care teams. They conduct in-home assessments, inventorying all medications, OTC, herbals, and supplements, which is entered securely into the HomeMeds digital platform. - Digital Platform Analyzes Medication Inventory for Risks
The HomeMeds platform automatically scans the complete medication inventory to identify potential risks. It checks for duplications, psychotropic drugs, NSAIDs, and other medication-related concerns, ensuring a thorough evaluation of all medications. - Pharmacist Reviews Medications and Provides Expert Recommendations
If issues are identified, the platform alerts a consulting pharmacist, who reviews the medications and offers expert recommendations for adjustments. - Physicians and Users Are Notified for Action
The prescribing physician and non-clinical consultants are notified of any risks. A report is generated, advising on necessary actions and proper medication use to ensure the safest treatment plan.
