Request for Proposal (RFPs) – Home Modification Contractor

Project Description

Partners in Care Foundation (Partners) is seeking a qualified contractor to complete home modifications related to the agency’s Older Adult Home Modification Program funded by the U.S. Department of Housing and Urban Development, which aims to conduct in-home assessments and provide home modifications to up to 302 older adults through a three-year grant period. This project will require collaboration with Partners’ project team and contracted Occupational Therapist. The home modification contractor will be expected to remodel areas of the home to meet specific accessibility needs and standards of older adults, including but not limited to installation of grab bars, wheelchair ramps, widening doorways, bathroom modifications, kitchen accessibility improvements, lighting enhancements, and mobility solutions.

Goals

  • To create a safe and accessible living environment for up to 302 older adults at risk for falls requiring home modifications to allow them to age in place.
  • To ensure that home modifications are responsive to older adults’ identified needs, and are completed with high quality standards, are durable, aesthetically pleasing, and cost-effective.

Scope of Work

  • Perform a thorough assessment of the area of the home identified for modification
  • Confer with Partners on the home modification recommendations made by the consultant Occupational Therapist
  • Complete up to 302 home modifications in Los Angeles County over the course of the three-year grant period
  • Ensure all materials and modifications adhere to safety and accessibility codes
  • Provide ongoing communication and progress reports through the project to Partners

Project Timeline

The project timeline is as follows:  

  • Project Start Date: 8/1/2024
  • Estimated Start Date for Home Modifications: 1/20/2025
  • Project End Date: 7/31/27

Evaluation Criteria

Proposals will be evaluated based on the following criteria:  

  • Experience and qualifications of the contractor (years in practice, certifications, specialized skills, and relevant experience). Preference will be given to contractors who have experience working with older adults.
  • Approach to the scope of work (methodology, plan, strategies).
  • References (professional references).
  • Budget alignment (cost-effectiveness and value for money).
  • Availability and timeline adherence (availability during the project period, ability to meet deadlines).
  • Compliance with professional standards (communication, ethical practices, documentation, timely invoicing).

Vendor Response Structure

Vendors should structure their proposals in the following manner:  

  • Executive Summary: A brief introduction to your company and experience in providing home modification services to older adults.  
  • Project Approach: A detailed outline of your approach to fulfilling the scope of work and achieving the goals. Include the earliest date you are able to begin work, and your ability to complete the required number of jobs by the project end date.
  • Qualifications: Information on your qualifications – including yourself and any key personnel, including relevant certifications, licenses, and experience, including any experience working with Partners or a similar community-based organization serving a similar population. 
  • Budget: Provide at least two examples of home modification jobs completed and how you ensured that the cost for completing the modifications was competitive and efficient. Please provide a typical pricing list for standard jobs, including grab bar and ramp installations, for example, and include information regarding your payment terms.
  • References: At least two professional references or case studies from previous clients.  
  • Business License, Certification(s) and Insurance: Documentation of your professional business license, certification(s) and insurance covering your professional liability for the scope of work.
    • All contractors hired by Partners are required to possess (or be willing to obtain) an independent business license

Proposals submitted after this date will not be considered.
