Job Title: Nurse Care Management Supervisor HCBA

Qualifications

  • Requires Bachelors’ degree in Nursing.
  • Requires an active California Registered Nurse license that is in good standing and active.
  • Requires 10 years of experience as Nursing shift supervisor, supervising nurse care management staff or complex case management staff.
  • Requires 10 years of experience working in either ICU, SAU, working with seriously ill, frail, or disabled adults.

Responsibilities    

  • Responsible for planning, organizing, and developing the principles and techniques employed in a comprehensive case management model for frail and disabled adults.
  • Ensures POT is person-centered and includes the individual’s goals and preferences.
  • Review and approval of all client service plans, and client cost monitoring.
  • Supervises nurse care management staff.
  • Reviews and approves timecards and mileage reimbursements.
  • Performs 90 day and annual performance reviews for all direct reports.
  • Analyzes social service/health operations internal and external to the program and recommending changes to improve service accessibility for clients.
  • Review of Nurse Care Manager compliance with implementing interventions and services utilizing community resources in a timely fashion in accordance with departmental policy.
  • Review of Nurse Care Manager compliance with maintaining and documenting regular contact with clients in accordance with departmental policy and providing ongoing support as needed.
  • Identifying, assessing, and responding to crisis situations in a timely fashion, with appropriate interventions.
  • Observing all legal, departmental, and/or hospital regulations.
  • Provides community relations by:
  • Developing and maintaining positive community interactions building referral relationships in the community.
  • Recognizing and resolving problems in a timely and appropriate fashion.
  • Maintain effective interpersonal communication skills by:
    • Using open and accurate verbal and written communication.
    • Recognizing and resolving conflicts or unusual situations.
    • Maintaining positive interpersonal relationships and team building within department and ancillary staff and with community partners.
