Project Description
Partners in Care Foundation (Partners) is seeking a licensed Occupational Therapist (OT) to provide services for the agency’s Older Adult Home Modification Program funded by the U.S. Department of Housing and Urban Development, which aims to conduct in-home assessments and provide home modifications to up to 302 older adults through a three-year grant period. This project will require collaboration with Partners’ project team and home modification contractor. The OT will be responsible for performing in-home assessments, presenting recommendations for home modifications, attesting to the quality and completion of the home modifications, and reviewing the work of Partners’ Certified Aging in Place Specialists (CAPS), once trained. The OT must have the means to be able to travel to in-home assessments in Los Angeles County.
Goals
- To assess and address the needs of up to 302 older adults at risk for falls requiring home modifications to allow them to age in place.
- To implement tailored interventions that facilitate functional independence in daily life.
Scope of Work
In Year 1:
- Conduct initial in-home assessments for up to 45 older adults to identify home modification needs
- Attend project team meetings to present home modification recommendations
- Maintain accurate documentation and reporting in Partners’ Home for Life platform (training to be provided)
- Consult with other professionals and family members, as needed
- Conduct post home modification follow-up in-home assessments for up to 45 older adults to ensure quality and completion of recommended modifications
In Years 2 and 3:
- Review the work and documentation of Partners’ CAPS-trained team members, who will be conducting the initial in-home assessments for up to 257 older adults
- Attend project team meetings to consult on CAPS team members’ home modification recommendations
- Maintain accurate documentation and reporting in Partners’ Home for Life platform
- Consult with other professionals and family members, as needed
- Conduct post home modification follow-up in-home assessments for up to 257 older adults to ensure quality and completion of recommended modifications
Project Timeline
The project timeline is as follows:
- Project Start Date: 8/1/2024
- Estimated Start Date for Initial Assessments: 1/6/2025
- Project End Date: 7/31/27
Milestones will be tracked and evaluated at quarterly intervals to ensure progress.
Budget
The budget for this project is capped at $75,000 (USD) over the three-year period. Please provide a detailed breakdown of your fees, including your hourly rate for assessments, local travel, meetings, and any additional fees associated with your services.
Evaluation Criteria
Proposals will be evaluated based on the following criteria:
- Experience and qualifications of the Occupational Therapist (years in practice, certifications, specialized skills, and relevant experience). Preference will be given to professionals who have experience working with older adults.
- Approach to the scope of work (methodology, plan, strategies).
- References (professional references).
- Budget alignment (cost-effectiveness and value for money).
- Availability and timeline adherence (availability during the project period, ability to meet deadlines).
- Compliance with professional standards (communication, ethical practices, documentation).
Vendor Response Structure
Vendors should structure their proposals in the following manner:
- Executive Summary: A brief introduction to your company and experience in providing occupational therapy services to older adults.
- Project Approach: A detailed outline of your approach to fulfilling the scope of work and achieving the goals.
- Qualifications: Information on your qualifications, including relevant certifications, licenses, and experience, including any experience working with Partners or a similar community-based organization serving a similar population.
- Please include a copy of your resume with your submission
- Budget: A detailed breakdown of your hourly rate related to conducting in-home assessments, local travel, and attending project meetings.
- References: At least two professional references or case studies from previous clients.
- Business License, Certification(s) and Insurance: Documentation of your professional business license, certification(s) and insurance covering your professional liability for the scope of work.
- All consultants hired by Partners are required to possess (or be willing to obtain) an independent business license
