Congratulations! Theresa Abston has been chosen as one of Partners’ two Employees of the Month.
Theresa started with Partners in Care Foundation in April 2020 and joined the Care Transitions team as a hospital coach in the Apple Valley supporting Providence patients. Theresa is very passionate about helping patients and ensuring that they obtain the resources and services they need. She has built an extensive compilation of resources in her area and has built rapport with local agencies in her community. She recently worked with an organization to obtain a donation of a Hoyer lift, rollator, hospital bed, shower chair, side hospital table, and wipes for a disabled veteran. Without Theresa’s dedication and persistence to find these services and equipment, this bedbound veteran would not have a way of getting out of bed.
Theresa is well deserving of this special recognition!
