Rachel Crizer has been chosen as one of Partners’ two Employees of the Month for May. She joined the Home and Community Based Alternatives (HCBA) Waiver Program in November of 2019 as a Case Manager. Rachel’s prior experience, knowledge of resources, and ability to easily connect with clients are tremendous assets to the HCBA program. She has excelled in gracefully managing a high-acuity caseload, while still making time to volunteer within the agency. Throughout her tenure at Partners, Rachel has delivered countless food boxes to HCBA clients throughout LA county. She also successfully navigated the Dignity at Home Fall Prevention Program which awarded her client a $5000 Stairlift. In addition, she stabilized a high-acuity client by effectively resolving communication barriers with his caregiver. Rachel continues to grow her skills and knowledge by participating in training and taking initiative during DHCS calls. Rachel’s fearlessness, organization skills, and assessment turn around rate are to be applauded. Congratulations!
