FOR IMMEDIATE RELEASE – SAN FERNANDO, CA (Dec. 13, 2019) — Partners in Care Foundation (Partners) would like to thank L.A. Care Health Plan for helping make possible a successful first year in California’s Health Home’s Program. Their generous gift of $60,000 enabled Partners to develop a care team, modify our IT systems, and train the 10 staff needed to serve HHP patients in Los Angeles County. The team has successfully implemented its action plan and achieved first-year target enrollments. Now funded through California’s Health Homes Program, our team can build on this initial success to serve more people.
Key learnings: homeless service providers need to be heavily involved and software is a key to success. Development of the software system – made possible through L.A. Care’s grant – enables us to provide care management, perform health risk assessments, craft health action plans, and report encounter data.
Congratulations to our Riverside, San Bernardino, and Los Angeles based Health Homes Program team on a successful first year! Under the direction of Anwar Zoueihid, Paty De Camp, Ester Sefilyan, Virginia Carone, Salman Mogaddedi, Patrina Croisdale and Raymond Morales, the team of Caleb Garmendia, George Severekyan, Danny Gomez, Michael Gamboa, Cynthia Rodriguez, Chad Taylor, Cody Ackley, Shawnte Moore, Summer Cox, Patricia Gomez, Laura Merodio, Carlos Azcona, Thomas Kostjal, Yuliya Zinkevich, Emilia Avagyan, Shad Cruz, Anna Kesablyan, Elena Merchand, Audrey Corley, Tim Morris and Shawn Callahan have labored mightily to set up a new service and hit first-year targets. We’re proud of your efforts and know that the people you serve appreciate all you are doing on their behalf.
