SAN FERNANDO, Calif. (Jan. 30, 2018)—The annual American Society on Aging (ASA) at the 2018 Aging in America Conference. The conference hosts more than 3,000 attendees annually from across the nation and abroad to learn, network and participate in the largest multidisciplinary conference covering aging and quality of life for older adults. Partners in Care Foundation is providing the following presentations at the ASA conference, which begins Monday, March 26, 2018 and will last through Friday, March 29, 2018.
Sustainable Care Transitions and Community-Based Care Management Contract Models
June Simmons, CEO; Sandy Atkins, MPA, VP, Strategic Initiatives, and Lisa Williams, MA, Director, Strategic Initiatives of Partners in Care Foundation believe community-based organizations (CBO) play an integral role in the challenging population health arena. Successful implementation of population health programs requires the adoption of broad population health strategies. We will provide the steps to developing a contract model between health systems and CBOs to establish sustainable care transitions and community care management models for high-risk Medicare beneficiaries. Strategies to Recruit and Retain Dedicated Participants and Leaders for Evidence-Based Programs
Dianne Davis, MPH, Senior Director, Health Self-Management Services of Partners in Care Foundation will discuss evidence-based health promotion programs have been proven successful for older adults and people with disabilities in managing their health, improving their quality of life and lowering healthcare costs. Learn about best practices for recruiting and retaining participants and workshop leaders, including how to develop creative marketing materials, use social media, build effective partnerships, engage under-served populations, and more. Patti League, RN, Wellness Specialist for A Matter of Balance, MaineHealth; Stanley Michaels, MFA, Injury Prevention Specialist, Hawaii State Department of Health; and Kathleen Zuke, MPH, Senior Program Manager, Center for Healthy Aging, National Council on Aging will join this panel on Monday, March 26, 2018 from 1:00 PM – 2:00 PM.
Statewide Delivery Networks: Where We Are and Where We’re Headed
Sandy Atkins, MPA, Vice President, Strategic Initiatives of Partners in Care Foundation will provide an overview of the pros and cons of community-based organizations establishing a statewide delivery network model for service delivery within healthcare partnerships. Speakers who have gone through the process of setting up statewide networks will share lessons learned from their experiences, as well as reflections and predictions on how this model might evolve in the future, as new policy and payment changes continue to shake up the health and long-term service system. Jonathan Lavin, MPA, CEO, AgeOptions; Jennifer Raymond, MBA, Chief Strategy Officer, Elder Services of Merrimack Valley, Inc.; and Nora Super, MPA, Chief, Programs and Services, National Association of Area Agencies on Aging (n4a) will also join this panel on Tuesday, March 27, 2018: 1:30 PM – 2:30 PM.
Community Based Organizations Networks for Healthcare: How Technology and Program Design Support Quality
Sandy Atkins, MPA, VP, Strategic Initiatives, Dianne Davis, MPH, Senior Director Health Self-Management Services, Christy Lau, MSSW, Program Director, and Ester Sefilyan, MSG, VP of Network Services of Partners in Care Foundation has been operating for the better part of three years. When a contract called for LCSW review of all cases, systems needed to be set up to make it easier for coaches to connect the assessment to a care plan. The contract also required a higher level of data reporting for evidence-based self-management programs. Learn how quality improvement systems were set up and supported with software to decrease cost and improve outcomes.
