Consultant pharmacists, Pharmaceutical Care Integration (PCI), have begun providing services to a number of Texas-based HomeMeds licensees experiencing difficulties finding local area pharmacists to perform medication reviews. In collaboration with PCI, Partners in Care Foundation has also instituted a pharmacists’ report log to standardize Medication Therapy Management problem/recommendation categories (including incorrect dose, adherence issues, adverse drug reaction, and therapeutic duplication) and to provide feedback on whether medication problems are likely to result in an individual being hospitalized or seeking emergency care.
